Supported Living Services
We provide tailored supported living services that encourage and motivate people to do as much for themselves as they are capable of, allowing them to gain both cognitive and practical skills.
Individuals facing a variety of challenges can now live the lives they envision for themselves and take advantage of the opportunities they have earned thanks to the supported living services we provide. Our services enable people to live more autonomous, choice-filled, and commanding lives.
Our key support workers make it a point to sit down and actively listen to each person in order to gain an understanding of their life goals. We work closely with the individual, their family, their circle of support, and any other healthcare professionals involved in the process of developing a support plan as well as achieving the goals outlined in the support plan. We take a results-oriented approach and provide support tailored to each individual’s needs.

Learning Disability
We provide assistance to people whose learning disabilities range from mild to moderate to severe. Because we are aware that every individual has their own set of requirements, our support plans will be individualised to meet the requirements of each person. Our individualised assistance places an emphasis on self-determination, active participation, and an enhanced quality of life.
We understand that you or they may have been taking care of things at home for a long time and that handing that responsibility over to someone else may be uncomfortable at first.
Physical Disability
Individuals with disabilities ranging from mild to severe and multiple disabilities can receive support from us. The appropriate level of assistance, which may include assistance with mobility, personal care, and communication, will be provided by our support workers. This assistance may also include assistance with other activities.
Autism
We provide assistance to people who have been diagnosed with autism spectrum disorder (ASD). An initial “autism profile” is compiled by our team so that we can cater our assistance to the specific requirements of each individual with autism.
Additionally, this evaluation assists us in elucidating the complexities of an individual’s condition, which can vary quite a bit from person to person.
Mental Health Needs
Individuals who come to us with moderate or mild mental health needs are eligible for our comprehensive mental health rehabilitation services.
Our primary goals are to help people feel better about themselves and to strengthen their emotional health. We have an amazing group of trained support workers who are used to helping people with a wide range of mental health problems, such as schizophrenia, personality disorders, and a lot of other conditions.

We make it a point to give each individual a bespoke care and support programme, one that they have input into and which they can co-create with members of our team and their loved ones.

- Input and lobbying are welcome.
- Acquiring skills for the home and for everyday life, such as cooking and cleaning.
- Getting out and about in the community.
- Keeping safe and healthy.
- Emotional support.
- Help to access other services.
The Outcomes Star Model is a framework whose principles are used to guide the development of our support plans. This model has been approved by the Department of Health. The Outcomes Star Model places an emphasis on the following:
- Managing finances and tenancy.
- Maximising independence.
- Taking care of one's mental health.
- Enhancing living skills.
